Getting your kid out the door in the morning - help! (OT)

I seem to be having a problem getting my daughter (age 4) out the door for daycare in the morning. She just doesn't seem to move in the morning unless I'm constantly on top of her to get dressed, brush her teeth, etc. I'm late 2-3 times per week, and that's when i can get her to eventually cooperate. Every now and then she just completely refuses to get ready for school. She loves school, so I know that's not the reason for her procrastination. I've changed my hours at work twice to try to accomodate her slow-to-go issues. I simply don't have anymore flexibility in that area. My tardiness will soon be a problem. I've tried many things to help her out. I've tried babying her with tons of help & support. I've tried taking video games & tv away in the evening (these things are already banned in the morning). We've tried rewards for getting out the door on time. None of it seems to help. During these days I end up screaming & yelling and acting like a mad woman. There's only been a few days I can remember where I felt like I was about to snap. This is not how I want to interact with my daughter and certainly not the behavior I want to model. I'm not out of control, but I'm walking the line. I hate feeling like I have to bully her into getting out the door, or being bullied by her. Sorry for being so long-winded, but I am desparate to hear from other moms as to how they get their uncooperative kids out the door in the morning. What methods do you employ? What constructive suggestions can you offer?
